AccessCare works differently from many community care agencies.
We focus on helping you get the support you need to stay in your own home and community.
We don’t offer direct care services ourselves. Instead, we help you plan a personalised care program, then source the services you need from a network of trusted providers.
Focus on living at home
AccessCare’s approach ensures you have choice and control. We want to empower you to be involved in your care as you choose to be. This might mean we provide services to achieve your goals for you, achieve them together with you, or build your capacity to achieve them yourself.
While residential care can be the right option for some people, our focus is on supporting people who have made the decision to stay at home for as long as possible.
Planning your support
When you become a client of AccessCare, you work with a case manager, who helps you identify your care goals and create a support plan that works toward achieving them. We then help you access the services you need from high quality healthhome and community care providers.
Throughout this process, you’re in control: you make decisions about your own care, and you can have as little or as much involvement as you wish in the management of your support plan.
At AccessCare we listen to your preferences and develop care plans that will achieve your goal as you intend — for example, if you do not have access to food and other daily living necessities. Depending on the level of involvement you wish to have, here are some of the options available:
- 6 week physiotherapy program to build strength and confidence to complete shopping independently
- Care worker can drive you to the shops and you complete the shopping together
- Care worker can shop on your behalf and bring groceries to your home
